Brazos Elementary School Medical Care and Protocol

 

Your Child will be sent to the school clinic in cases of illness or accident.  First aid will be administered to cuts, scratches, and minor playground accidents.

 

In emergency situations which require that your child be taken immediately to a doctor or a medical facility, you will be notified of the transfer.  Meanwhile,  school representative will remain with your child until your or your designated person arrives.

 

To prevent exposing other students to disease, your child should not come to school if (s)he has the following symptoms:

  • vomiting or diarrhea the morning before school

  • an undetermined rash

  • a fever of 100.4 F. or above within the past 24 hours.

Guidelines for Exclusion from School:

 

        1.    You will be notified to pick up your child from school if (s)he complains of:

                    a fever of 100.4 F. or higher                                             

                    a suspected contagious disease                                                   

                    vomiting                                                                         

                    diarrhea

                    an undetermined rash

                    being too ill to remain in school

        2.  Facilities for keeping your sick child at school are limited, therefore, IT IS IMPORTANT THAT YOU PICK   

             UP YOUR CHILD AS SOON AS POSSIBLE.

        3.  In cases where your child cannot remain in school, you or the person you have designated
            on your child's Emergency Information Card will be notified to pick him/her up.  When
            your child leaves school with your designated person, (s)he must sign a release accepting
            responsibility for your child.
                                  

 

 

Short Term Medications

 

This permit is to filled out by the parent/guardian for any medication given in school for less than 15 days.

You must indicate the time, the amount, and the number of days the medication is to be given at school.  Send only enough medication needed for use at school.  MEDICATION CANNOT BE TRANSPORTED BY STUDENTS TO AND FROM SCHOOL.

Long Term Medications

 

This permit is to be completed by the parent/guardian and the physician for any medication that is given in school for longer than 15 days. 

 

  • ALL MEDICATIONS MUST BE PROVIDED WITH PROPER PHARMACY LABEL.

  • MEDICATION MUST BE BROUGHT TO SCHOOL BY THE PARENT/GUARDIAN.

  • FOR SAFETY REASONS, NO MEDICATION CAN BE SENT HOME WITH THE STUDENT.

ANTIBIOTICS

 

***ANY ANTIBIOTIC GIVEN TWICE A DAY SHOULD BE GIVEN AT HOME. 

ALL MEDICATION MUST BE SENT IN THE ORIGINAL CONTAINER WITH THE PROPER PHARMACY LABEL.

 

OVER THE COUNTER MEDICATIONS

 

***Over the counter medications must be age appropriate for the student.  Please check the expiration date on the medication before sending it.  If it is expired, the medication cannot be given.  Requests afro doses larger than the manufacturer's recommendation will not be given unless a physician's request accompanies the medication.  The number of days the student is to receive the medication must not exceed the manufacturer's recommendation.

 

 

 

Permit for Asthma Medication

 

For Inhalers

This permit must be completed by the physician.  The parent must also sign the permit concurring with the physician's recommendation. 

 

No student can hand carry an asthma inhaler unless the physician indicates this on the permit.

 

Nebulizers

Brazos Elementary has a nebulizer for students that use single vial asthma medications.  Depending upon the number of days the medication will need to be used at school, either the long or short term medication permit will need to be completed.  The parent/guardian is responsible for supplying the nebulizer kit including tubing along with the medication.

IMMUNIZATIONS

 

State of Texas and Brazos ISD Requirements for Immunizations:

 

The State of Texas requires that if a child is behind on immunizations the child must be as current on immunization as possible before entry into the school setting. 

 

Students transferring from a school outside Texas MUST present immunization records at the time of enrollment.  Students transferring from a district within Texas can be enrolled on a 30 day provisional enrollment until records form the previous school are received.  If immunization records are not received by the end of the 30 day provisional enrollment period, it is the responsibility of the parent/guardian to provide Brazos I. S. D. with these records.  The student will be excluded from attendance until the immunization record is received.

 

The following is a schedule of immunizations required to be received:

 

DPT/DTaP/Td

Ages 4-6:

  • Must have proof of 5 doses of vaccine UNLESS the 4th dose was given on or after the 4th birthday, then only 4 doses are required.

Ages 7 and Above:

  • Must have at least 3 doses of vaccine with the last having been given on or after the 4th birthday.

  • One dose of Td is required 10 years after the last dose of DTPaP/Td.

Polio

  • Must have proof of 4 doses of polio vaccine UNLESS the 3rd dose was given on or after the 4th birthday.

Measles/MMR

  • Must have received one dose of MMR vaccine on or after the 1st birthday.  A second dose must be received on or before age 5.  This second dose may be part of a second MMR.

Hib

 

Ages 4 and Under

  • Must have proof of a completed 4 shot series OR at lease one dose administered at 15-59 months of age.

Pneumococcal (PCV7)

  • All students through 59 months of age planning to attend pre-kindergarten or early childhood must provide proof of a complete 4 shot PCV series with the last dose given on or after 12 months of age OR if the series is incomplete or the child has never received a dose of PCV7, at least one dose of PCV7 must be received after 24 months of age.

Hepatitis A

  • All pre-kindergarten and early childhood students must provide proof of having received 2 doses of hepatitis A vaccine.

Hepatitis B

  • ALL students in Pre-K through 12th must provide proof of 3 hepatitis B vaccines.

Varicella

  • ALL students in Pre-K through12th must have received one dose of varicella vaccine on or after the 1st birthday if the student has not had chickenpox.

  • If a student has NOT had chickenpox and has NOT received one dose of varicella vaccine by age 13, two doses of varicella vaccine will be required.

  • If your child has had chickenpox, a parent/guardian must sign a verification for stating the student has had the illness indicating the approximate date of the illness.

TB Test (per Brazos I. S. D. local policy)

  • First time enrollees into a public school setting (Pre-K and K) entering the district at the beginning of a school year MUST have proof of a TB test with the results having been received within one year prior to entry into Brazos I. S. D.

  • All students Pre-K through12th transferring into the district from an in-state or out-of-state school must present proof of a TB test with results having been received during the course of the student's education.  This proof must be resented at the time of enrollment into Brazos I. S. D. or a new TB test  must be obtained prior to attendance.

Pre-K, Kindergarten, and transfer students from out of state:  ALL IMMUNIZATION RECORDS MUST BE REVIEWED BY THE NURSE PRIOR TO ENTERING THE CLASSROOM SETTING ON THE STUDENT'S FIRST DAY OF ATTENDANCE AT BRAZOS ISD.
